Abstract

Penelitian ini bertujuan untuk menganalisis jenis cacat dominan pada produksi air minum gelas 240 ml di PT Swabina Gatra serta memberikan usulan perbaikan berbasis analisis risiko. Metode yang digunakan adalah kombinasi Statistical Process Control (SPC) dan Failure Mode and Effect Analysis (FMEA). SPC digunakan untuk memantau kestabilan proses melalui check sheet, diagram Pareto, dan P-Chart, sedangkan FMEA digunakan untuk menentukan prioritas risiko berdasarkan nilai Risk Priority Number (RPN). Hasil analisis menunjukkan cacat lid cup bocor sebesar 78,19% dan miring sebesar 21,81%. Penyebab utama berasal dari mutu material, kondisi jig, dan kalibrasi mesin. Usulan perbaikan difokuskan pada pengendalian material, perawatan mesin, dan pelatihan operator.
