Abstract

This review aimed to assess the consistency of the associations between IL-6 and hs-CRP and T2DM complications, as well as the readiness of the evidence for risk stratification. The systematic review followed PRISMA 2020 guidelines. PubMed, ScienceDirect, and SpringerLink were searched for observational studies published between 2021 and 2026. Risk of bias was assessed using JBI instruments, and results were synthesized narratively according to SWiM guidelines. Eleven studies met the inclusion criteria. IL-6 showed associations with heart failure, retinopathy and macular edema, cardiovascular autonomic neuropathy, peripheral artery disease, and diabetic foot. hs-CRP was linked to mortality, diabetic kidney disease, and retinopathy, though independent associations were inconsistent regarding cardiovascular events and non-proliferative diabetic retinopathy (NPDR). Heterogeneity in study design, assays, matrices, and the control of confounders precluded meta-analysis. In conclusion, IL-6 and hs-CRP capture distinct inflammatory dimensions and potentially complement conventional risk assessment, yet they are not sufficiently validated for use as standalone tests.
